The discharge papers say "return home with assistance." The hospital bed is needed for the next patient. And somebody has to be at the house tonight to make sure Mom does not fall on the way to the bathroom, takes the right pills at the right time, and actually eats. REACH Home Care provides hospital-to-home care throughout the Tri-Cities so that somebody can be a trained, licensed caregiver, often starting the same day your family member leaves Kadlec, Trios, Lourdes, or a rehab facility.
We are a locally owned, Washington State-licensed in-home services agency (DOH license IHS.FS.61064639) based in Kennewick. We provide the non-medical side of recovery: the supervision, personal care, meals, mobility help, and medication reminders that home health nurses and therapists do not cover between their visits, and that families struggle to provide around work and their own lives.
Why the first days home matter most
Most readmissions happen in the first week, and most of them are preventable: a fall on the first night, a missed or doubled medication, dehydration, an infection nobody noticed, or simply not being able to get up to eat. A caregiver in the home during the first 72 hours changes those odds. Our care coordinators build post-hospital plans around that window, with heavier coverage at the start that tapers as your loved one regains strength.
What REACH hospital-to-home care includes
Same-day or next-day start: we can often meet you at the home the day of discharge, and we can begin planning while your loved one is still in the hospital or rehab.
Discharge instruction follow-through: caregivers keep the written discharge plan in view and help with activity limits, wound-site precautions, diet orders, and follow-up appointment reminders.
Medication reminders: new prescriptions and changed doses are where post-hospital errors happen. Caregivers prompt each dose, organize pill boxes with the family, and report anything unusual.
Mobility, transfers, and fall prevention: help getting in and out of bed, to the bathroom, and around the house, including gait belt and Hoyer lift transfers for clients who are weak after surgery or illness.
Personal care: bathing, dressing, toileting, and grooming while incisions heal and strength returns, handled with care for dignity and any post-surgical precautions.
Meals and hydration: cooking to the diet your discharge plan calls for, encouraging fluids, and making sure meals actually happen.
Overnight and 24-hour care: an awake caregiver overnight during the highest-risk nights, or round-the-clock coverage in shifts for clients who cannot yet be alone.
Coordination with home health and family: we work alongside visiting nurses and therapists, keep notes on what we observe, and call the family when something changes.
Household help and errands: laundry, dishes, groceries, and pharmacy pickups so the house keeps running during recovery.
Common situations we help with
Hip and knee replacement recovery, heart surgery and cardiac events, stroke, pneumonia and other infections, falls and fractures, cancer treatment, and any hospital stay for an older adult who lives alone or whose spouse cannot safely provide the care. We also help families who are transitioning a parent from a skilled nursing or rehab facility back home, which carries many of the same risks as a hospital discharge. If your loved one needs a longer recovery, the same caregivers can continue as ongoing personal care or respite care.
How our caregivers are trained
Every REACH caregiver completes Washington's 75-hour Home Care Aide training and state certification exam at the REACH Training Center in Kennewick, with skills-lab practice on transfers, gait belts, and safe body mechanics. All caregivers pass a Washington State background check, and our care coordinators brief each caregiver on the specific discharge plan before the first shift.

How to get started, even before discharge
1. Call us at (509) 491-1733. If your loved one is still in the hospital, call now. We can plan with you and the discharge planner before the discharge date is set.
2. Free assessment. A care coordinator meets you at the hospital, the home, or by phone, reviews the discharge plan, and writes a recovery care plan with you.
3. Home preparation and caregiver match. We help you clear walkways, set up the bedroom and bathroom, and assign a caregiver briefed on the plan.
4. Heavier support early, then taper. Coverage is front-loaded for the first days and adjusted as strength returns. No long-term contract.
We accept private pay and long-term care insurance, and we bill many insurers directly. If Medicare home health has been ordered, we work alongside it rather than replacing it.
Frequently asked questions about post-hospital home care
How quickly can post-hospital care start?
Often the same day as discharge if you call while your loved one is still in the hospital, and usually within 24 hours otherwise.
Is this the same as Medicare home health?
No. Home health is skilled medical care (nursing, therapy, wound care) ordered by a physician and delivered in short visits. REACH provides the non-medical, hands-on care between those visits: supervision, personal care, meals, mobility help, and medication reminders. Many families use both.
Can you help after a rehab or skilled nursing discharge?
Yes. A return home from rehab carries the same fall and medication risks as a hospital discharge, and we plan for it the same way.
Do you provide overnight or 24-hour care after surgery?
Yes. Awake overnight care during the first nights home is one of the most common requests we get, and we provide 24-hour care in shifts for clients who cannot be left alone.
Can caregivers help with transfers?
Yes. Caregivers are trained in gait belt transfers, transfer boards, and Hoyer lifts, and we assess the safest method during the first visit.
Do you accept long-term care insurance?
Yes. We work with most long-term care insurance policies and can bill many insurers directly.
